A large cyst may cause ovarian torsion (twisting) resulting in pain in the pelvis and abdomen that comes and goes. An ovarian cyst may burst (rupture) or leak and cause severe pelvic and abdominal pain.

Ultrasound scans use high-frequency sound waves to accurately reproduce an image of your pelvic organs and can help determine the size, number, location and type of cyst.
Most ovarian cysts are asymptomatic, painless and harmless. In fact, it is not uncommon for cysts to resolve spontaneously. If the cysts are small, benign and cause little to no pain, they can be safely monitored with serial follow-up ultrasound scans and blood tests (ovarian tumour markers) a few months later.
Oral analgesia can be used to reduce pain but they do not have any effect on the growth of the ovarian cyst.
Hormonal treatment in the form of the oral contraceptive pills (OCPs) prevent ovulation and therefore the chance of new cysts developing in future menstrual cycles.
Surgery is recommended by an ovarian cyst gynae if the symptoms are severe or getting worse, if the cyst is large or increasing in size and numbers or if there is a suspicion of it being cancerous.
